- fix: processing &#number; codes. Number is an Unicode character code.

  No, TrvHtmlImporter does not support Unicode yet, it converts these

  characters to ANSI using RVStyle.DefCodePage for conversion

- fix: correct scaling of images when only width (without height) or

  only height (without width) is defined.
