Probably you have two versions of RichViewActions installed, and they

conflict with each other?

Or, may be, you uncommented USERVADDICT3 in RichViewActions.inc when

installing RichViewActions, and now it's commented again (for example, you

have upgraded RichViewActions without recompiling packages)
